This Privacy Policy explains how Form Intel, LLC, a South Carolina limited liability company ("form.digital," "we," "us," or "our"), collects, uses, shares, and protects information through the website located at digital.formintel.co and related inquiry forms, program applications and partner accounts (collectively, the "Site"). form.digital is the digital-services department of Form Intel, LLC.
This Privacy Policy should be read together with our Terms of Use (digital.formintel.co/terms), Cookie Policy (digital.formintel.co/cookies), and Website Disclaimer (digital.formintel.co/disclaimer). By using the Site, you acknowledge that you have read and understood this Privacy Policy.
1. Information We Collect
1.1 Information You Provide. When you submit a build-intake form or otherwise contact us, we may collect: (i) contact details, including your name, email address, phone number, and organization; (ii) project details, including service lane, budget range, timeline, project description, current tools, and any links you provide; and (iii) any other information you choose to include in your submission or correspondence.
1.2 Information Collected Automatically. When you visit the Site, our servers and hosting infrastructure (currently Vercel) may automatically collect limited technical data, including: (i) IP address; (ii) browser type and version; (iii) operating system; (iv) referring URL; (v) pages visited and time spent on each page; and (vi) date and time of access (collectively, "Log Data").
1.3 Cookies and Similar Technologies. The Site uses only essential cookies required for the Site to function, including those set by our hosting provider. For full details on the cookies we use, how to manage them, and your rights regarding them, see our Cookie Policy at digital.formintel.co/cookies. If we add non-essential cookies in the future, we will update the Cookie Policy and, where required by applicable law, obtain your consent before placing those cookies.
1.4 Program and Partner Information. When you apply for a case study or ministry program, we collect the organization and project context you choose to provide. When you create a referral-partner account, we collect your profile details, verification and account-security events, program acknowledgment, optional profile photo, referral introductions, and confirmed referral and reward status. Ministry inquiries may reveal your affiliation with a religious organization. Please share organizational needs rather than private information about congregants, counseling, health or children. Do not submit passwords, full financial account details or government identifiers through these forms.
1.5 Referral Contacts and Photos. Share another person’s contact details only when you have permission to make the introduction. Partner profile photos are stored privately and are not published as a public directory. The portal re-encodes uploaded photos and removes embedded metadata. Banking setup and payment execution are not available in this release; we do not request bank or tax details through the partner forms.
2. How We Use Your Information
We use the information we collect for the following purposes: (i) to respond to your inquiry and scope your project; (ii) to follow up regarding a potential or active engagement; (iii) to operate, secure, maintain, and improve the Site; (iv) to comply with applicable legal obligations; and (v) to enforce our rights arising from any agreements between you and us; and (vi) to manage partner accounts and program applications, confirm introductions and referral progress, send transactional notifications, prevent duplicate submissions, and maintain security and activity records.
3. How We Share Your Information
3.1 Service Providers (Sub-Processors). We share information with third-party service providers that assist us in operating the Site and conducting our business, solely to the extent necessary for them to perform services on our behalf. As of the Effective Date these include: (i) Vercel — website hosting and infrastructure; (ii) Resend — delivery of transactional and lead-notification email; (iii) Anthropic, PBC — the AI model provider that processes the messages you send to our on-site AI assistant ("ladson ai") in order to generate responses; and (iv) monday.com — the platform we use to receive and manage inquiries and leads submitted through the Site. These providers are permitted to use your information only to provide services to us and are bound by their own privacy and security commitments. We may add or change sub-processors as our tooling evolves and will update this list accordingly.
When partner access is enabled, Supabase provides the partner database, authentication and private photo storage. Program submissions and necessary partner/referral details are synchronized to monday.com; Resend delivers account and submission notifications. Passwords, authentication tokens, bank details and private photo-access URLs are not sent to monday.com or included in owner-notification emails.
3.2 Legal Requirements. We may disclose your information if required to do so by law, regulation, legal process, or governmental request, or if we believe in good faith that disclosure is necessary to protect our rights, your safety, or the safety of others.
3.3 Business Transfers. In the event of a merger, acquisition, reorganization, or sale of all or a portion of our assets, your information may be transferred as part of that transaction. We will notify you of any such change by updating this Privacy Policy.
3.4 No Sale of Personal Information. We do not sell, rent, or trade your personal information to third parties for their marketing purposes. We have not sold personal information in the preceding twelve (12) months.
4. Data Retention
We retain inquiry and program submissions, partner account and referral records, and related personal information for as long as reasonably necessary to evaluate and pursue a potential engagement and to meet our legal, accounting, or record-keeping obligations. Account-security and activity records may also be retained to investigate abuse, reconcile record changes and meet applicable record-keeping obligations. Once the information is no longer needed for these purposes, we will delete or de-identify it. You may request deletion of your information at any time as described in Section 6.
5. Data Security
We implement reasonable administrative, technical, and physical safeguards designed to protect your information from unauthorized access, use, alteration, or destruction. However, no method of transmission over the Internet or method of electronic storage is completely secure, and we cannot guarantee absolute security.
6. Your Rights and Choices
6.1 General Rights. Regardless of your location, you may at any time: (i) request access to the personal information we hold about you; (ii) request correction of inaccurate information; (iii) request deletion of your information; or (iv) decline to provide information, though doing so may limit our ability to respond to your inquiry. 6.2 Identity Verification. Before processing any access, correction, or deletion request, we will verify your identity by confirming that the request originates from the email address associated with your original intake submission or correspondence. If you contact us from a different email address, we may ask you to provide sufficient identifying information to match your request to the data on file. We will not fulfill a request until verification is complete.
6.3 Response Timeline. We will acknowledge receipt of your request within five (5) business days and will complete the request within thirty (30) calendar days of receipt. If we require additional time due to the complexity of the request or the volume of requests received, we will notify you in writing of the extension and the reason for it before the initial thirty-day period expires. Any extension will not exceed an additional thirty (30) calendar days.
6.4 Rights Under the General Data Protection Regulation (GDPR). If you are located in the European Economic Area, the United Kingdom, or Switzerland, the following additional provisions apply:
(a) Legal Basis for Processing. We process your personal data on the following bases: (i) legitimate interest — to respond to your inquiry, evaluate a potential engagement, and operate and improve the Site; and (ii) legal obligation — to comply with applicable laws.
(b) Additional Rights. In addition to the rights in Section 6.1, you have the right to: (i) restrict or object to our processing of your personal data; (ii) request portability of your personal data in a structured, commonly used, and machine-readable format; and (iii) withdraw consent at any time where processing is based on consent, without affecting the lawfulness of processing before withdrawal.
(c) Data Controller. Form Intel, LLC is the data controller responsible for your personal data. You may contact us at privacy@formintel.co.
(d) Supervisory Authority. You have the right to lodge a complaint with a data-protection supervisory authority in the jurisdiction where you reside or work, or where an alleged infringement occurred.
(e) International Transfers. Your personal data is transferred to and processed in the United States. We rely on appropriate safeguards, including standard contractual clauses where applicable, to protect personal data transferred outside the EEA, UK, or Switzerland.
6.5 Rights Under the California Consumer Privacy Act (CCPA). If you are a California resident, the following additional provisions apply:
(a) Categories of Information. In the preceding twelve (12) months, we have collected the following categories of personal information: (i) identifiers (name, email, phone number); (ii) commercial information (project details, budget range); and (iii) internet or electronic network activity information (Log Data). These categories were collected from you directly and from automatic collection as described in Section 1.
(b) Right to Know. You have the right to request that we disclose the categories and specific pieces of personal information we have collected about you, the categories of sources, the business or commercial purpose for collecting it, and the categories of third parties with whom we share it.
(c) Right to Delete. You have the right to request that we delete the personal information we have collected from you, subject to certain exceptions.
(d) Right to Non-Discrimination. We will not discriminate against you for exercising any of your CCPA rights.
(e) How to Submit a Request. To exercise your rights under the CCPA, contact us at privacy@formintel.co. We will verify your identity as described in Section 6.2 before fulfilling your request. The response timeline in Section 6.3 applies, provided that CCPA requests will be completed within forty-five (45) calendar days where applicable law requires that timeline.
7. Children's Privacy
The Site is not directed to individuals under the age of sixteen (16). We do not knowingly collect personal information from anyone under 16. If we learn that we have collected personal information from a child under 16, we will take steps to delete that information promptly. The Site may contain links to third-party websites or services. We are not responsible for the privacy practices of those third parties. We encourage you to review the privacy policies of any third-party site you visit.
